Cultural Competence Health Practitioner Assessment
The Cultural Competence Health Practitioner Assessment (CCHPA) is a cultural competence self-assessment tool for practitioners. The CCHPA takes 20 minutes to complete. Upon completion of the CCHPA, you will receive response fields that identify you at an awareness, knowledge, or skill level for each of the six subscales. Depending on your response pattern, you will be provided with a listing of resources such as web-based journals, textbooks, multimedia materials, and suggested learning experiences intended to support your individual growth.The CCCHPA was developed by the National Center for Cultural Competence (NCCC) at the request of the Bureau of Primary Health Care, Health Resources and Services Administration, U.S. Department of Health and Human Services.

Cultural Competence Self-Assessment Checklist for Primary Care Practitioners
This 37-item checklist is designed for practitioners to self-assess their level of cultural competence. It provides concrete examples of the kinds of beliefs, attitudes, values and practices which foster cultural and linguistic competence at the practitioner level. This instrument was developed by Tawara D. Goode at the National Center for Cultural Competence.

Cultural Competence Self-evaluation Form (CCSE)
The Cultural Competence Self-evaluation form (CCSE) is designed as a self-assessment instrument uses a five-point Likert scale. The instrument is based on work by Anthony Marsella and colleagues in Atlanta, Georgia.

Cultural Competency Assessment Tool
This resource kit was prepared by the Alberta Children’s Hospital/Child and Women’s Health Diversity Program Coordinator, and the Program Facilitator, Mental Health Diversity Program (2002-2007). The information is intended to assist health professionals in providing culturally competent care to individuals and families from diverse cultural backgrounds. This kit was produced in response to a need identified by the diversity program coordinators within the Alberta Health Services that health care professionals need easy access to information in order to enhance their skills in providing culturally competent care to individuals and families from diverse cultural backgrounds.

Cultural Competence Self-Assessment Checklist for Practitioners Providing Support to Individuals and Families Affected by Sudden Infant Death Syndrome and Other Infant Death
This 29-item checklist is designed for practitioners to address their awareness and sensitivity to the importance of cultural diversity and cultural competence surrounding sudden infant death syndrome or other infant death (SIDS). This instrument was developed by Tawara D. Goode at the National Center for Cultural Competence. This document is available in English and in Spanish.

Cultural Competency: A Self-Assessment Guide for Human Service Organizations
This guide was prepared by Hieu Van Ngo for Canadian Heritage (Calgary Office) in 2000. This guide examines cultural competency at the organizational level. It also advocates that cultural competency is an important learned skill set that is relevant to all service providers, regardless of their cultural backgrounds or practice settings. The guide points out the changes in Canadian demography, government legislation, socio-economic reality of culturally diverse people in Canada. It also presents 9 tools to help organizations identify their progress as well as needed improvements to achieve cultural competency. In addition, it provides definitions of key terms at the beginning, and closes with a list of resources relating to cultural competency.

Cultural Competency Assessment Tools
This tool is designed for behavioural healthcare providers who are serving multicultural populations. It was developed by Center of Excellence in Culturally Competent Mental Health, in Orangeburg, NY. It can be used as self-assessment, care coordination and as a guide to monitoring change over time. The results of the assessment can be used in the context of quality assurance/quality improvement activities. The scale is most effective when an agency has regularly updated information about the cultural groups of its service users.

This 40-item checklist is designed for practitioners to self-assess their awareness and sensitivity to the importance of cultural diversity and cultural competence surrounding behavioural health issues for children, youth and their families. This instrument was developed by Tawara D. Goode at the National Center for Cultural Competence.

Cultural Competence Self-Assessment Checklist for Practitioners Providing Early Childhood Intervention
This 49-item checklist is designed for practitioners to address their awareness and sensitivity to the importance of cultural diversity and cultural competence surrounding early childhood intervention. This instrument was developed by Tawara D. Goode at the National Center for Cultural Competence. This document is available in English and Spanish.
